Acid-base Changes following Exchange Transfusion with Citrated Blood

چکیده

The acid-citrate-dextrose (ACD) solution used to preserve blood consists in effect of 2 parts trisodium citrate to 1 part citric acid. It forms a buffer solution such that the pH of bank blood less than 5 days old is, in our experience, 6 6 ± 0 * 2. It can be predicted, therefore, that infusion of ACD blood must lead to an initial acidosis, and that this will in due course be replaced by a metabolic alkalosis as citrate is metabolized to form 3 moles of sodium bicarbonate for each mole of trisodium citrate retained. In the individual subject, the rates at which these two effects operate will determine the severity of the initial acidosis, and the stage at which the later alkalosis develops.

Control of acidosis during exchange transfusion with citrated blood.

In a previous paper (Calladine et al., 1965) it was shown that during exchange transfusion with citrated blood an acidosis often developed in babies transfused on the first day of life, but not in those transfused on the third day or later. The acidosis in the babies concerned was sometimes severe, and was occasionally associated with clinical symptoms. It was concluded that in performing excha...

Hyperbilirubinemia Following Exchange Transfusion with G-6-PD Deficient Donor Blood

Background: The incidence of glucose-6-phosphate dehydrogenase (G-6-PD) deficiency in Iran is estimated at 10-14.9%.  The donor blood in blood banks is not screened routinely for this enzyme deficiency and such blood may be used for neonatal exchange transfusion. Objective: To study the effect of G-6-PD deficient blood in neonatal exchange blood transfusion. Methods: In a prospective study, serum...

Evaluation of the Severity and Duration of Thrombocytopenia following Exchange Transfusion in neonatal hyperbilirubinemia

Background: Infant jaundice is one the most common causes of hospitalization in infant in the first month of birth, which is defined an abnormal increase in blood bilirubin levels. Exchange transfusion is the recommended treatment for neonatal jaundice who do not respond to phototherapy and experience dangerous complication of jaundice and signs of kernicterus. However, this treatment may lead ...

The Comparison of Acetic Acid and Strontium Chloride Procedures for Extraction of Hemin

Abstract Background and objectives: Hemin is a porphyrin compound derived from hemoglobin, the precursor of other porphyrin hemoglobin derivatives and the raw material of Hematin. Since hemin is widely used in medicine, we decided primarily to synthesize this substance in Laboratory and to determine the best way of hemin extraction from untransfused and expired blood units. Materials and Method...
